INVITATION TO TENDER AND EXPRESSION OF INTEREST FOR EXECUTION OF 2020 CAPITAL PROJECTS
The Federal Medical Centre, Umuahia in fulfillment of her mandate to provide Quality and Qualitative Health Care Services, intends to use her budgetary allocation for the procurement of Works, Goods and Services towards achieving her mandate as appropriated by the Federal Government in the 2020 Capital and Zonal Intervention Budget.
Accordingly, the Federal Medical Centre, Umuahia invites qualified, interested and eligible Companies (Manufacturers, Suppliers, and Contractors etc.) to tender for the following procurement of Goods, Works and Services:
The scopes of this procurement are as follows;
CATEGORY A: WORKS
Lot 1: Reactivation of Oxygen Machine/Upgrade.
Lot 2: Construction of In-vitro Fertilization Building.
Lot 3: Construction of Multipurpose Hall/Upgrade.
CATEGORY B: PROJECT CONSULTANT
Lot 4: Project Consultant for the Screening and Awareness Training in Diabetes and Blood Pressure Management in Umuahia North and South and Ikwuano L.G.A Abia Central Senatorial District, Abia State
3. ELIGIBILITY REQUIREMENT
Interested bidders shall present the following documents in their technical Tender:
i. Evidence of Business Name or Certificate of Incorporation issued by Corporate Affairs Commission (CAC) including forms CAC 1.1 or CAC 2 and CAC 7 where applicable (Form CAC2, 2.1 and 7 are still applicable for the companies that have not made any changes on their Membership and Directorship Since incorporation)
ii. Evidence of Company’s Income Tax Clearance Certificate (or Personal Income Tax Clearance Certificates of all the Partners) for the last three (3) years 2017, 2018 and 2019 valid till 31st December, 2020
iii. Evidence of current Pension Compliance Certificate valid till 31st December, 2020.
iv. Evidence of current Industrial Training Fund (ITF) Compliance Certificate valid till 31st December, 2020.
v. Evidence of Registration on the National Database of Federal Contractors, consultants and services providers by submission of Interim Registration report (IRR) expiring on 30/06/2020 or valid certificate issued by the BPP.
vi. Evidence of current Nigeria Social Insurance Trust Fund (NSITF) Compliance Certificate valid till 31st December, 2020.
vii. Verifiable documentary evidence of at least three (3) similar jobs in the last five (5) years including letter of Awards, Valuation Certificates Job completion and photographs of the projects.
viii. Evidence of Financial capacity to execute the project by submission of Reference letter from a reputable Commercial Bank in Nigeria, indicating willingness to provide credit facility for the execution of the projects when needed.
ix. Company’s Audited Accounts for the last three (3) years: 2017, 2018 and 2019
x. Sworn affidavit:
– that firm is not bankrupt
– that none of the Directors is an ex- convict or bankrupt;
– disclosing whether or not any officer of the relevant committees of Federal Medical Centre, Umuahia or the Bureau of Public Procurement is a former or present Director, shareholder or has any pecuniary interest in the bidder and to confirm that all information presented in its bid are true and correct in all particulars;
– that the Company would comply to Local Content Policy;
xi. Company’s Profile with Curriculum Vitae of key staff to be deployed for the project, including copies of their Academic /Professional qualification and registration with professional bodies (COREN, QSRBN, ARCON, CORBON etc.)
xii. List of Plants/Equipment with proof of ownership/lease (where applicable).
xiii. Category B: Evidence of Firm’s registration with relevant professional body(ies);
xiv. All documents for submission must be transmitted with a covering /forwarding letter under the Company /Firm’s letter Head paper bearing amongst others, the Registration Number(RC) as issued by the Corporate Affairs Commission (CAC), Contact Address, Telephone number (preferably GSM No) and e-mail address. The letter head paper must bear the names and Nationalities of the Directors of the company on the bottom of the page, duly signed by the authorized officer of the firm.
NOTE: All pages of document submitted must be authenticated by the Company CEO or his/her representative, failure will lead to disqualification.
4. COLLECTION OF TENDER DOCUMENTS
Category A: The bid document can be obtained from the procurement unit, Federal Medical Centre, Umuahia upon an evidence of payment of Ten Thousand Naira (N10,000.00) only non-refundable fee paid into Federal Medical Centre Umuahia’s Remita Account in any reputable Commercial Bank in Nigeria.
Category B: Firms interested in Category B will not pay at this stage, until after shortlisting.
5. SUBMISSION OF TENDER DOCUMENTS
Prospective bidders are to submit two (2) hard copies of the Technical and Financial Bids packaged separately in sealed envelopes and clearly marked ‘‘Technical Bid’’ or ‘‘Financial Bid’’ with the name of the bidder, address and phone number. Put the two sealed envelopes together in a bigger sealed envelope with name of bidder, address and phone number addressed to;

The bid document should be deposited in the tender box specified for the project at the office of the Head Procurement, Federal Medical Centre, Umuahia not later than 12:00 noon on Monday 22nd June, 2020. Company name and address should also be written at the reverse side of the envelope. Prospective Bidders should ensure they sign the register provided on submission of bid documents. Late submission will be returned un-opened.
Interested firms are to submit two (2) bound of Expression of Interest (EoI) documents separated by dividers and arranged as indicated above. The documents should be submitted in a sealed envelope and addressed as indicated above, and clearly marked with (the name of the project and the Lot number). Furthermore, the reverse of each sealed envelope should bear the name and address of the firm and drop in the designated Tender Box not later than 12:00 noon on Monday, 25th May, 2020.
6. OPENING OF EXPRESSION OF INTEREST AND TECHNICAL BIDS
The EOI and Technical Bids will be opened immediately after the deadlines for submission at 12:00 noon on Monday, 25th May, 2020 and 12:00 noon on Monday 22nd June, 2020 respectively, at the Board Room 2nd Floor, New Administration Complex. All bidders or their representatives, relevant professional bodies and NGOs are invited to witness the public opening exercise.
7. GENERAL INFORMATION
i. Bids must be in English Language and signed by an official authorised by the bidder.
ii. All costs will be borne by the bidder.
iii. Category A: Only Technically qualified bidders will be contacted at a later date for financial bids opening, while financial bids of un-successful bidders will be returned un-opened;
iv. Category B: Only shortlisted firms will be invited at a later date for collection of Request for Proposals;
v. This advertisement shall not be construed as a commitment on the part of the Centre or shall it entitle tenderers to make any claims or seek any indemnity from the Centre by virtue of their response to the Advertisement.
vi. All CAC, PENCOM, ITF, NSITF and TAX certificates maybe referred to the Security Agencies, Corporate Affairs Commission, Federal Inland Revenue (FIRS), National Pension Commission and other relevant bodies for verification
vii. The Federal Medical Centre, Umuahia reserves the right to annul the process at any time without incurring any liability in accordance with section 28 of the Public Procurement Act 2007.
